To request a copy of a shot record, please call the office. Records must be picked up in person and there is a $5.00 charge per patient record.

Need to schedule an appointment? We do our best to inform you throughout the year via mail as well as at check-out when your child is due for a return visit. Please call the office to schedule an appointment or you can email us, and we will get back to you as soon as possible.
